动态增强MRI定量评价强直性脊柱炎骶髂关节炎症活动度的价值
[摘要] 目的 探讨动态增强MRI（DCE-MRI）定量评价强直性脊柱炎（AS）炎症活动度的价值。 方法 收集我院2016年1月～2018年1月24例AS患者的临床和影像学资料，24例经临床病理确诊的AS患者行双侧骶髂关节DCE-MRI检查，运用动态增强专用血流动力学软件测量双侧骶髂关节滑膜病变区定量参数：容积转移常数（Ktrans），采用t检验比较分析滑膜炎症不同活动度组间Ktrans值的差异。 结果 24例AS骶髂关节滑膜病变Ktrans值为（1.32±0.08）mL/min，8例滑膜炎症为重度，16例滑膜炎症为轻度。滑膜炎症重度组Ktrans值明显高于轻度组（t=4.07，P<0.01）。 结论 DCE-MRI的Ktrans值可用于反映AS患者骶髂关节的炎症活动程度。

[Abstract] Objective To explore the value of dynamic contrast enhanced MRI（DCE-MRI） in the quantitative evaluation of inflammatory activity in patients with ankylosing spondylitis（AS）. Methods The clinical and imaging data of 24 patients with AS treated in our hospital from January 2016 to January 2018 were collected. 24 patients who had been clinically and pathologically diagnosed as AS underwent bilateral sacroiliac joint DCE-MRI examination. Dynamic contrast enhanced special hemodynamic software was used to measure the parameters of the bilateral sacroiliac synovial lesion area： volume transfer constant（Ktrans）. The t test was used to compare the difference of Ktrans between groups of different inflammatory activity. Results The Ktrans value of 24 cases with AS sacroiliac joint synovial lesions was（1.32±0.08） mL/min， synovial inflammation of 8 cases of which were severe， while that of 16 cases of which were mild.
