肌钙蛋白I和D-二聚体在慢阻肺急性加重期患者血浆中的表达及其临床意义
[摘要] 目的 探讨肌钙蛋白I和D-二聚体在慢阻肺急性加重期患者血浆中的表达及其临床意义。 方法 选取2016年5月～2018年6月期间206例我院接收的慢性阻塞性肺疾病患者，206例患者中，稳定期患者97例，AECOPD患者109例，于入院当天检测肌钙蛋白I和D-二聚体水平。 结果 AECOPD组患者血浆cTnI水平及D-二聚体在慢阻肺急性加重期患者血浆中的表达明显高于COPD稳定组，差异具有显著性（P<0.05）;Ⅲ级组患者血浆D-D水平、cTnI水平较Ⅱ级组和Ⅰ级组明显要高（P<0.05），Ⅱ级组患者D-D水平、cTnI水平较I级组患者明显要高（P<0.05）;将AECOPD组患者cTnI水平与D-D水平进行直线相关分析后显示，二者呈显著性正相关（r=0.825，P<0.05）。 结论 COPD急性加重期患者的cTnI及D-D水平均明显升高，且不同病情时cTnI及D-D水平存在明显差异，二者联合检测可作为AECOPD临床诊断和疗效评价的重要指标。

[Abstract] Objective To investigate the expression and clinical significance of troponin Ⅰ and D-dimer in plasma of patients with acute exacerbation of chronic obstructive pulmonary disease. Methods A total of 206 patients with chronic obstructive pulmonary disease admitted in our hospital from May 2016 to June 2018 were eolled. Of the 206 patients， 97 were stable patients and 109 were AECOPD patients. The levels of troponin I and D-dimer on the day of admission were detected. Results In AECOPD group， the plasma cTnI level and the expression of D-dimer in plasma of patients with acute obstructive pulmonary disease was significantly higher than that of the COPD stable group（P<0.05）. The difference was significant. The plasma D-D level and cTnI level in the group Ⅲ was significantly higher than that in the group Ⅱ and Ⅰ（P<0.05）.
