经阴道四维超声造影评估子宫输卵管通畅性的研究
[摘要] 目的 分析经阴道四维超声造影（TVS 4D-Hycosy）在子宫输卵管通畅性评估中的临床价值。 方法 回顾性分析2014年8月～2015年8月延安市人民医院不孕症患者89例，新型微泡型造影剂（Sono Vue）造影基础上采用TVS 4D-Hycosy实时观察子宫输卵管形态、造影剂流动及弥散等超声影像特征，评估输卵管的通畅性及不良反应。 结果 89例不孕患者的173条输卵管的4D-Hycosy造影图像显示，输卵管通畅61条，表现为输卵管柔软，造影剂快速进入盆腔并迅速弥散；输卵管通而不畅78条，表现为输卵管局部纤细或增粗，造影剂缓慢进入盆腔，弥散延迟；输卵管不通畅34条，表现为双侧输卵管中断或不显影。不良反应包括轻度疼痛46例，中度疼痛38例，迷走神经反应或疼痛需要观察4例。 结论 TVS 4D-Hycosy可立体动态观察子宫输卵管形态结构及走行，有效判断输卵管的通畅性，可作为临床不孕症患者输卵管通畅性判断的一种安全的筛选方法。

[Abstract] Objective To analyze the clinical value of transvaginal four-dimensional hysterosalpingo-contrast sonography （TVS 4D-Hycosy） for evaluation of the unobstruction of uterus fallopian tube. Methods The clinical data of 89 cases of patients with infertility in Yan"an People"s Hospital from August 2014 to August 2015 were analyzed retrospectively. 4D-Hycosy was used for real-time observation of ultrasonic imaging characteristics， including uterus fallopian tube feature， contrast agent stream and diffusion and so on， and assessment of the unobstruction of uterus fallopian tube and the adverse reaction， on the basis of the new type microbubble contrast agent （Sono Vue）.
