我院肠杆菌属ESBLs耐药情况及CTX-M-1、SHV型耐药基因研究

【摘 要】目的:了解2016-2018年本院肠杆菌属分布情况及部分耐药基因分型。方法:利用WHONET5.6软件分析耐药率变化。针对产ESBLs酶菌株进行CTX-M-1、SHV型耐药基因分型。结果:2016-2018年期间共得到肠杆菌属124株;大多数菌株来自神经外科(19%),标本类型主要为痰(56%);肠杆菌属对青霉素类、头孢菌素类、单环β-内酰胺类抗菌素均存在较高耐药率(>20%),对复方新诺明的耐药率明显呈上升趋势。有46(37.1%)株对ESBLs耐药菌株,主要为CTX-M-15型(21.7%)。结论:建议通过更多的研究建立一套完整的基因耐药监测体系。

Abstract:Objective To understand the distribution of Enterobacter and the types of drug resistance genes in the isolated specimens of our hospital from 2016 to 2018. Methods:The change of drug resistance rate was analyzed by WHONET5.6 software. CTX-M-1 and SHV-type genotyping were performed on strains producing ESBLs.Results: A total of 124 strains of Enterobacter were obtained during 2016-2018;Most of the strains were from neurosurgery(19%).The type of specimen was mainly sputum (56%); Enterobacteriaceae have high resistance rate to penicillins,cephalosporins and monocyclic β-lactam antibiotics, which is more than20%.The resistance rate of Nomin is rising clearly. There were 46(37.1%)strains resistant to ESBLs, mainly CTX-M-15(21.7%).Conclusions:It is suggested that a complete set of genetic drug resistance monitoring procedures should be established through more research.
