血管性认知障碍与代谢综合征的研究进展
【摘要】 目前, 我国发病率最高的为缺血性脑血管病, 在人们物质生活水平不断提高的前提下, 代谢综合征对人们的影响也越来越深入, 代谢综合征使血管性认知障碍的发生风险明显增加, 其是一组以肥胖、高血压、糖尿病和血脂异常为主要表现的症候群, 而胰岛素抵抗(IR)是代谢综合征发生的主要病理生理学基础。代谢综合征及其各个组成成分与血管性认知障碍均有密切关系。目前研究并不能充分证明代谢综合征对血管性认知障碍的风险发生率大于其各个组成成分的总和。代谢综合征以中心性肥胖、糖尿病或糖调节受损、高血压、血脂异常以及胰岛素抵抗为共同病理生理基础, 其对大脑认知功能有广泛的不良影响, 但在何种阶段表现为何种认知异常特征, 尚未有明确的研究结论, 而且在临床治疗中对认知功能损伤的康复评估也未得到应有的重视。代谢综合征为缺血性脑血管病的危险因素, 且与血管性认知障碍密切相关。本文就血管性认知功能与代谢综合征的关系、二者的发病机制及相关概念综述如下。

【Abstract】 At present, the most prevalent disease in China is ischemic cerebrovascular disease. With the continuous improvement of people’s living standard, the influence of metabolic syndrome on people is more and more thorough. Metabolic syndrome increases the risk of vascular cognitive impairment, and it is a group of symptoms characterized by obesity, hypertension, diabetes mellitus and dyslipidemia. Insulin resistance is the major pathophysiological basis of metabolic syndrome. Metabolic syndrome and its components are closely related to vascular cognitive impairment. The current study does not adequately demonstrate that the risk of metabolic syndrome for vascular cognitive impairment is greater than the sum of its individual components.
